MEDICAL PLAN
Genesis Concepts' Group Medical Plan is offered to all full-time eligible employees. We have selected a Humana PPO with a $25/40 standard co-payment 100/70 percent plan for office visits.  The Plan pays 100% if you use a participating provider and 70% if you use a nonparticipating provider.

Genesis Concepts will contribute 50% of the employee's medical premiums, which will include a Vision 471 rider. The Prescription Drugs coverage is assigned to one of the four different levels with corresponding co-payment amounts at participating pharmacies. We will also contribute 25% of the
employee's dental premiums.

SUPPLEMENTAL PLAN
Genesis Concepts has selected AFLAC as our supplemental insurance provider.  We offer the Section 125 Cafeteria Plan.  This plan allows eligible employees to redirect a portion of their salaries to pay for qualified benefits through pre-tax salary reduction rather than with after-tax dollars. It doesn't sound like much, but it can save employees a lot of money! Our cafeteria plan service is designed to help employees make the most of tax savings.
